HEALTH CHECK FORM FOR AUGUST

Please arrive 15-20 minutes for the start of session. We are required to have all skaters, parents and coaches enter the arena at the same time as a group. DOORS will be Closed when the session starts!
One Parent/Guardian per Skater (Clarington Guidelines)
Please submit you Health Check before 3 pm.
Why 3 pm? One of our volunteer executive members is reviewing and completing a summary form required upon request by the Municipality.
